Alcohol and Drug Survey

This survey will be used to create Personalized Alcohol and Drug Use Feedback for you so that ADCAPS can provide you with the best service possible. The personalized profile can only be as accurate and useful as the responses you provide. We would like your contact with ADCAPS to be helpful and interesting, so please be as honest as possible in your answers.
Your survey is only seen by the professional staff of ADCAPS. It is not part of your University record or your housing record, if you live on-campus.
