Alcohol and Drug Assessment
This assessment takes approximately 20 minutes. Allow yourself adequate time and consider whether your environment provides the privacy you need.
Based on the answers you give us, we create your Personalized Alcohol and Drug Use Feedback Profile. The Personalized Feedback Profile is only as accurate as the responses you provide. Be as honest as possible.
The information you provide in this assessment is totally confidential. Your assessment information is only seen by the professional staff of the Wildcat Wellness Center. The information you provide does not become part of your University record or your housing record if you live on-campus. It is not shared with the Office of Housing or Student Affairs. If you are being referred by the Ellensburg Municipal Court, the information is not shared with the court.
Review our confidentiality statement which is on the next page. You must verify that you have read the Statement of Confidentiality and the Notice of Privacy Practices before starting the assessment.
At the end of the assessment, you have the option of immediately viewing your Personalized Feedback Profile by clicking on the button to view results.
