Follow-Up Survey
The Follow-Up Survey you are about to begin will take you approximately 15-20 minutes. Please allow yourself adequate time to complete the assessment. Also consider whether or not the environment in which you are taking this assessment provides the amount of privacy you need to feel comfortable.
This Follow-Up Survey is essential to assess the effectiveness of the services we provide. Your responses on this Follow-Up Survey are only seen by the professional staff of OPPEN House and are completely confidential just like the original assessment you filled out.
This Follow-up is not part of your university record, if you live on-campus your responses on this follow-up will not result in any additional requirements, or referrals. Your individual responses will not be shared with housing, student life, or any other party except for those involved with OPPEN House, without your written consent.
At the end of the Follow-up Survey, we will notify housing of your completion
